FDA 510(k) Biocompatibility Support for Medical Devices
Preparing a 510(k), assessing a device change or answering an FDA Additional Information request? Start with a scoped evidence review and a written list of gaps and next documentation steps.
Clarify what your FDA biocompatibility file supports and what needs attention
MedDev Advisory provides independent ISO 10993 evidence and documentation support. An initial non-confidential fit check is free; document review and drafting are paid work with scope, deliverables and timing agreed in writing. This does not include full FDA submission execution or laboratory testing, and does not imply FDA endorsement or guaranteed acceptance.
First 510(k) Submission
You need an FDA-facing biocompatibility section that is clearly structured and aligned with current guidance expectations.
FDA Additional Information Request
You already submitted, but FDA flagged missing rationale, incomplete chemistry framing, or weak support for waived endpoints.
Material or Supplier Change
The device changed and the biocompatibility section now needs a targeted reassessment tied to the final finished configuration.
Need to Rely on Existing Data
You want to use literature, chemistry, predicate logic, or existing test data instead of defaulting to a full testing battery.
What a focused FDA evidence review could produce
A manufacturer is preparing a 510(k) and has test reports from an earlier device version plus newer chemistry information for the current finished device.
1. Confirm the device-to-evidence link
Review question: Do the tested samples represent the current materials, manufacturing, sterilization and final finished configuration?
Possible output: Device-to-evidence comparison and missing-input list.
2. Map evidence to FDA questions
Review question: Which endpoints are supported, which rationales need strengthening, and where must chemistry or toxicology be connected?
Possible output: Endpoint table with evidence, gaps, assumptions and waiver rationale.
3. Create the reviewer-facing route
Review question: What needs to change in the 510(k) section or in a point-by-point response before the file is ready for review?
Possible output: Prioritized action register and scoped narrative or response structure.
This fictional example does not establish evidence applicability, a testing decision, a waiver or FDA acceptance. Exact outputs depend on the agreed scope and available inputs.
Common reasons FDA pushes back on biocompatibility sections
These are recurring patterns in weak FDA sections. Resolving them early can prevent avoidable delays and clarify what evidence is actually needed.
Final-Device Representation Is Unclear
Testing on raw materials, an earlier configuration or a differently processed sample may not represent the current finished device. Thin material, supplier, manufacturing or sterilization descriptions make that gap harder to resolve.
Endpoint and Existing-Evidence Rationale Is Weak
“Not applicable” is not a scientific justification. Each endpoint needs a traceable reason, and literature or older reports need a clear connection to the current device and current review question.
Chemistry and Toxicology Are Not Integrated
When extractables or chemistry data exist, the section still needs a clear bridge to biological risk and endpoint conclusions.
The Reviewer Cannot Trace the Answer
The right data can still be difficult to evaluate when rationale is scattered or generic. A response should connect each FDA question to the supporting evidence and the exact revised section.

How FDA 510(k) support is typically structured
The work is scoped around the actual device, current evidence package, and whether the goal is a fresh submission section or a response to reviewer comments.
Free Initial Fit Check
Share a non-confidential device brief and the question you need to resolve. Paid review starts only after scope, deliverables, fee and timing are agreed.
Map the Evidence Strategy
Existing testing, literature, chemistry data, and waiver pathways are assessed against the endpoint logic relevant to the device.
Draft the Reviewer-Facing Section
The biocompatibility summary, endpoint table, and supporting rationale are structured so the review path is clearer and more defensible.
Finalize or Remediate
The draft is refined with your feedback, and if FDA has already responded, the work can be focused on the exact deficiency points raised.
Questions teams usually ask before starting
What do you need from us to start?
For the free fit check, share a non-confidential device description, intended use, target submission and question or deadline. Supporting files are requested later through an agreed channel.
Can you help after an FDA Additional Information request?
Yes. MedDev Advisory can scope a point-by-point response to the biocompatibility questions raised, using your existing file and identifying where supporting evidence is missing.
Do you replace biological testing?
No. The goal is to determine what can be justified with existing evidence and what still requires testing. Strong support can reduce unnecessary testing, but not eliminate required evidence.
Is this only for 510(k), or also De Novo and PMA?
The page is focused on 510(k), but the same support model can also help teams preparing De Novo or PMA biocompatibility sections where the scientific logic needs to be reviewer-facing and defensible.
Need a stronger FDA biocompatibility section?
Send a non-confidential device brief and the question or deadline you need to address. The initial fit check is free; any document review, drafting and response work is scoped and agreed separately.
